High School Transcript Requests

Transcript requests must be submitted in writing.  Please allow up to two weeks for transcripts to be processed.    

Current students:
To request a transcript, please e-mail your counselor and let her know that you need a transcript.  Please let us know where to send it (including mailing address).  

Former students:
To request a transcript, please fill out this form:

In order to process the transcript, we need the form with your signature stating that we can release your transcript. 
 You can e-mail the form to the school counselors, or you can fax it to the high school office at 937-289-7019. 

If you need a transcript from Urbana University, you must request it directly from the university.  Colleges will not accept your high school transcript with Urbana courses listed in lieu of a transcript directly from Urbana.  Click here for the transcript request form from Urbana University. 

To request an education verification:
Please direct the company to send us the request in writing.  We cannot provide a verification without a records release that you have signed stating that we can release your information.  An e-signature is acceptable on education verification forms.  You can e-mail the form to the school counselors, or you can fax it to the high school office at 937-289-7019.
